Cardinal Hill Rehabilitation Hospital’s Stroke Program was created in 1978 with one central goal: to return clients who have experienced a stroke to meaningful lifestyles in the shortest time possible.

We help clients reach their goals to function independently through benchmark rehabilitation services.

An interdisciplinary team specializing in the unique needs of stroke clients provides rehabilitative care.

The team includes:

The Client & Family
Physiatrist (Physician specializing in Rehabilitation Medicine)
Rehabilitation Nurse
Physical Therapist
Occupational Therapist
Speech-Language Pathologist
Case Manager/Social Worker
Dietitian
Therapeutic Recreation Specialist
Psychological Services

Internal Medicine consultation, urology consultation, pastoral counseling, vocational rehabilitation counseling and social services are also available, as needed.

Individualized Treatment Plans

The rehabilitative plan of care for our clients recovering from a stroke may include any or all of the following services:

Adjustment counseling
Aquatic therapy
Bowel & bladder retraining
Community living skills group with community re-entry classes
Community referrals

Diet & medication retraining
Driving education
Evaluation of home environment

Home management training
Neuropsychological testing
Physical, Occupational & Speech-Language Therapy

Stroke education classes
Support groups
Swallowing evaluation & retraining

Therapeutic recreation
Vocational rehabilitation counseling

Family Involvement

Family involvement is vital to a client's success in rehabilitation. We encourage a family member, a friend or other caregiver to participate in therapy sessions, training sessions and support groups

Personalized Care

Each client receives a personalized rehabilitation plan. The goal of that plan is to restore physical and social independence to clients experiencing a debilitating crisis.

Each plan takes into account the client's previous abilities, health, the area of the brain affected by the stroke and the extent of the injury. Clients work to improve mobility, self-care, communication and socialization.

Hospital length of stay and discharge schedules are dependent upon the client's progress, the home situation and the level of family involvement.

Discharge                                                                 

The client's relationship with Cardinal Hill does not end with the discharge from the inpatient program. Each client's primary care physician will receive a copy of the Discharge Summary. Outpatient services can begin immediately.

These may include a Day Program for therapy, selected therapy services, vocational rehabilitation training, home health services and Adult Day Health. For the client's convenience, referral to a provider closer to home may be made.

The client also may be scheduled to return for Outpatient Clinic visits with his/her Cardinal Hill physician. During these visits, progress is assessed and changes are made in the home program or equipment, as needed.
