There are 14 Case Managers at Cardinal Hill Rehabilitation Hospital all of whom are Masters level Social Workers who are certified at that level. Case managers serve as an advocate for the patient and family and take responsibility for ensuring that patients receive appropriate and timely services needed to facilitate and maximize the patient’s rehabilitation outcomes. Case managers coordinate the patient’s rehabilitation program working with physicians, therapists, nurses and other rehabilitation specialists to provide a comprehensive program of services during inpatient rehabilitation. Case managers also work with federal, state, community, social and medical organizations to identify resources to help with discharge planning and continued care after discharge.
