EASTER SEAL CAMP KYSOC

 

HEALTH CARE PLAN

 

1)                      COMPOSITION OF HEALTH CARE STAFF

 

a)          Off Site Camp Physician

 

(1)     On-call Physician licensed in the State of Kentucky

(2)     Agreement made annually with physician.

(3)     Standing orders, procedures for health screening, and camp health care plan reviewed and approved annually by Camp Physician.

b)          Camp Nurse

(1)      On-site Registered Nurses (RNs) registered in State of KY

(2)      Nurse is on duty at all times when camp is in session.

 

(a)     Responsibilities of Camp Nurse

 

1.        Work understanding orders of physician (See attached Treatment procedures).

2.        Implements camp arrival and camp departure Health screening as outlined in screening procedures for campers and staff.  (See attached CAMP KYSOC HEALTH SCREENING OF CAMPERS/STAFF ON OPENING &CLOSING DAYS.)

3.        Carries responsibility for health education.  (See Attached SUGGESTED INFORMATION TO BE COVERED DURING CAMP KYSOC STAFF ORIENTATION.)

4.        Responsible for handling of emergencies

5.        Responsible for orientating staff regarding use of universal precautions and camp’s Bloodborne Pathogens Plan.

6.        Responsible for implanting Post-Exposure Plan for HBV with record keeping.

7.        Observes for signs of illness and determines when physician is needed.

8.        Isolates a camper or staff member in the health lodge as indicated until seen by a physician.

9.        Assists Camp Director in reviewing health records and medical histories of camper and staff.

10.     Inventories and maintains all health service supplies.

11.     Responsible for all medications (prescriptions, over-the-counter, and vitamins) of campers and staff.

12.     Keep camper files during each camp session.

13.     Maintains infirmary 24 hours a day.

14.     Keeps necessary records and reports

a.        Treatment records

b.       Medication Records

c.        Camp arrival screening and departure screening

d.       Records necessary Occupational Health and Safety Act (staff related/ worker’s compensation)

e.        Completes and submits camp health and accident insurance forms to office

f.         State report forms for child and adults abuse

g.       Assists Camp Director with completing and submitting Worker’s Compensation forms

h.       Assists with reports forms for child or adult abuse

 

15.     Observes sanitation and safety practices throughout camp, making regular visits for checking, and reports to Camp Director.

 

2)                      HEALTH CARE FACILITY AND SUPPLIES

 

a)       The camp health lodge consists of a reception room with file cabinet to contain camp records and camper file in alphabetical order, a camp telephone, and desk for writing reports. A separate bedroom with adjacent bathroom provides living accommodations for the two Health Care Staff. Three other rooms, an examination room, an isolation room, and bathroom are available in the health lodge.

b)       Carroll County Ambulance Service, Phone no. (502-732-7038), is located next to the hospital and can be used for emergencies.

c)       It is the responsibility of the nurse to make sure there are current health and accident insurance forms and directions to the hospital in the reception room of the health lodge. The Camp Business Manager can obtain additional insurance forms from our office.

d)       It is the responsibility of the last person using the camp emergency vehicle to make sure the van is properly filled with gas before returning it to the marked parking space near the circle drive in the front of the Camp Office.

 

3)       WRITTEN HEALTH RECORD REQUIREMENTS FOR CAMPERS AND STAFF

 

a)       Health history updated every 6 months and health examinations are required for every camper and staff member. Campers must have their examination done by a physician within 24 months of camp. Staff must have their physical examination done by a physician within 24 months of the beginning of camp employment. Staff must also show evidence of a negative TB test or chest x-ray within 12 months prior to camp.

b)       Signed consent for emergency care is required for all campers and staff. Campers may not be admitted to camp without having this signature. Staff must also consent to emergency care. If they are under 18 years of age, they must have parental consent.

c)       Camper health history and recent health examination forms will be kept in camper files, which will be kept in the file cabinet in the health lodge. Staff history/health examination forms will be kept in a separate file in the same file cabinet. (Both camper and staff forms will be alphabetized)

d)       Camper files will be returned to the office at the end of each session. Staff files will be returned to the office at the end of camp.

e)       Camper and staff health treatments will be recorded and maintained in the bound health log in accordance with the American Camping Association standards—no lines should be skipped and all entries should be written in ink and signed with date and time of treatment.

 

4)       PROCEDURES FOR HEALTH SCREENING.

 

The Camp nurse shall supervise screening of campers and staff at Camp Kysoc for the current camp season in accordance with written procedures approved annually by the Camp Physician. (see CAMP KYSOC HEALTH SCREENING OF CAMPERS/STAFF ON OPENING/CLOSING DAYS).

a)       The nurse is responsible for supervising the screening of campers and or/ staff and must have in hand the individual’s health history and health examination form.

b)       The camper history/exam form should be checked to see if the parent, guardian, or adult responsible for the individual has signed it, within three months of the camp attendance.

c)       The screening shall occur within 24 hours of the person’s departure for camp or arrival in camp. The departure screening shall be completed and dated the day before closing day of each session. A camper’s physical condition shall be checked and recorded at this time.

d)       In screening a person, identify any observable evidence of illness, disability or communicable disease, noting in appropriate place.

e)       Review health history/exam form for: current medications and/or treatment procedures, dietary restrictions, allergies, and physical limitations. If medication dosage on examination form or prescription bottle differed from parent/guardian instructions, the caregiver must complete “special consideration for medication” forms. This authoritizes the nurse to dispense medication per caregiver instructions. The nurse shall contact M.D. if further clarification is needed.

 

f)        Record on the individual’s Camp Health Screening form upon arrival and departure:

(1)     Temperature

(2)     Condition of Skin, Throat, and Ears

(3)     Medications turned in. (noted on medication log rather than screening form)

(4)     Condition of appliances (wheelchair, braces, etc.)

(5)     Special health requirements not noted on the camper/staff health history or physical examination forms.

g)       Any evidence of illness, communicable disease, or disability shall be evaluated and treated by the Camp Nurse. The camp nurse is responsible for reporting such evaluation and the outcome of their evaluation to the Camp Director as soon as possible. This most definitely includes any evidence to local Protection Service workers immediately.

 

5)       FIRST AID/EMERGENCY CARE

 

a)       The Camp Nurse, as well as first aid certified staff, are responsible for administering first aid. Certified staff will provide emergency care until the individual is in the care of the camp nurse. Staff will receive training for emergencies during the pre-camp orientation/training period.

b)       First aid may be administered in accordance with current Treatment Procedures and the current edition of the American Red Cross First Aid.

c)       After fist aid is given in an emergency, the Camp Physician or local hospital emergency room physician shall be contacted for follow-up care. If it appears essential to the Camp Nurse or Camp Director that the individual be moved for additional medical supervision, the camp will notify the Carroll County Memorial Hospital by telephone (732-4321) asking them to notify our Camp Physician, Dr. Cecil Martin.

d)       The Camp Nurse is to report any cases of accident, injury or fatality to the Camp Director. Notification of parents or authorities is at the discretion of the Camp Director. Communication with the parents may be delegated by the Camp Director to the Camp Nurse in specific situations.

e)       Documentation of such case is made within the health logbook, and an accident report is to be completed by the camp counselor and/or the Camp Nurse.

 

6)       ROUTINE HEALTH CARE

 

a)       Every counselor assigned to campers is responsible for routine health care as reviewed in the Staff Manual and pre-camp training.

b)       The Camp Nurse, in cooperation with the Camp Director, will explain the health care program and will participate in instruction during pre-camp staff training.

c)       The Camp Nurse will at all times act under the guidance and advice of the Camp Physician and will call upon him as necessary and as directed in the TREATMENT PROCEDURES approved by him.

d)       The Camp Nurse or designee will observe sanitation and safety practices throughout camp and complete a sanitation checklist per camp directors request or as needed. (See attached CAMP KYSOC SANITATION CHECKLIST)

 

e)       Medication Policies:

(1)     The Camp Nurse inventories and maintains health service supplies.

(2)     Narcotics and medications are to be stored under lock in the health lodge, locked in the medication cabinets, the refrigerator, and/or in the medication wagon.

(3)     DIRECT ALL questions regarding medications the camper has brought to camp, to the Camp Nurse. ALL medications must be brought to camp in prescription bottles labeled with the persons name, the name of the drug, the dosage, the time medication is to be taken, the doctor’s name, the prescription or RX number and the pharmacy supplying the drug (both staff and camper medications)

f)        See CAMP KYSOC HEALTH SCREENING OF CAMPER/STAFF ON OPENING/CLOSING DAYS regarding change in dosage.

 

g)       Medication Policies:

(1)     The Camp Nurse inventories and maintains health service supplies.

(3)     At any time there is a question as to the medication the camper has brought to camp, the Camp Nurse will contact the camper’s physician. ALL medications must be brought to camp in prescription bottles labeled with the persons name, the name of the drug, the dosage, the time medication is to be taken, the doctor’s name, the prescription or RX number and the pharmacy supplying the drug (both staff and camper medications)

(4)     The following administration system, by our nurses, will best support camp programming and safety when giving medications to campers and staff.

(a)     Medications will be stored according to camper groups. Staff meds will be stored in one group.

(b)     Each camper’s medication(s) will be stored in a clear plastic zip lock bag with his/her name and camper group on it.

(c)     Each day, the Camp Nurse will organize meds to be given to prior to meals and bedtime or other medication times as directed by the camper’s physician via the prescription directions on the bottle.

(d)     Medication will be divided into pill dosage boxes and stored in the camp’s medicine wagon—ready for the nurse to take to the dining hall pavilion, or village area.  Campers who arrive from residential care facilities usually have their medication already prepared in daily unit dose boxes.   After each session pillboxes to be sanitized in dishwasher.

(e)     The Camp Nurse will administer medication directly to individual campers at the pavilion immediately preceding meals and at bedtime, the only exception to this procedure will be for camper groups going on a cookout overnight, then the nurse will deliver to the overnight site.

 

h)       The nurse will report health problems of residents to the Camp Director with recommendations and will not communicate with parents directly unless requested to do so by the Camp Director.
